Fill & Sign options greyed out

I'm trying to sign a W-9. I could fill out the top part of the form OK, but when I went to add a signature, all of the options in the Fill & Sign panel are greyed out. Do I need to do something special to make them available? I'm using Acrobat Pro 11.0.04 on an iMac running OSX 10.8.5.
