When "Preparing a Form" in Adobe Pro DC, I used to have a check box below the options to "Select a file or scan a document to begin" that would allow me to say whether or not the document would include signatures.  The checkbox is no longer there and Adobe is not allowing me to add signature blocks (the way I used to) and date boxes.  Help!
